Successful transitions happen when youth, families, schools, service systems, commuity-based organizations, employers, and other transition partners work together. This page brings together tools, strategies, and frameworks developed by the Transition Improvement Grant (TIG) and through the collective efforts of CCoT teams across Wisconsin to support your team’s journey toward improved outcomes for youth with disabilities.

The County Community on Transition (CCoT) Team Playbook is a practical guide created by the Transition Improvement Grant (TIG). Built from evidence-based, research-based, and promising practices, this playbook translates successes into actionable strategies. Whether starting a new team or revitalizing an existing one, the playbook offers adaptable practices to keep CCoT Team’s focused and results-driven.

Many of the strategies, ideas, and resources featured within the Playbook were developed in collaboration with the TIG CCoT Pilot Project teams as we identified local needs, implemented innovative solutions, and strengthened community partnerships to improve transition processes for youth with disabilities and their families. These strategies, ideas, and resources highlight the creativity, dedication, and collaboration taking place across communities while providing practical tools and promising practices that can be adapted by other teams.

The downloadable CCoT Team RESET PowerPoint is designed to help teams pause, reflect, and refocus their work throughout the year. This resource guides teams through reviewing current practices, identifying priorities, celebrating progress, and determining next steps. Whether used during annual planning, after team membership changes, or when teams need to re-energize their efforts, the RESET PowerPoint provides a structured process to help teams stay aligned, focused, and moving forward toward improved outcomes.

Wisconsin Community on Transition Innovation Plan

The Wisconsin Community Transition Innovation Plan (WiCTIP) is a comprehensive strategic planning online tool designed to provide a framework for the development of a highly effective County Community on Transition (CCoT) Team.

This strategic process aims to support teams in developing a collaborative interagency team, analyzing local and statewide data to inform data-driven decision making opportunities, and developing a sequence of services designed to foster robust collaboration with youth and their families.
