County Communities on Transition Resources

Successful transitions happen when youth, families, schools, agencies, organizations, employers, and other community partners work together. This page brings together resources, tools, and examples developed by the Transition Improvement Grant (TIG) and through the collective efforts of CCoT teams across Wisconsin.

Many of the resources featured here were created by TIG CCoT Pilot Project Teams as they identified local needs, implemented innovative solutions, and strengthened community partnerships to improve transition processes for youth with disabilities and their families. These resources highlight the creativity, dedication, and collaboration taking place across communities while providing practical tools and promising practices that can be adapted by other teams.

CCoT Toolkit

The County Community on Transition (CCoT) Toolkit was developed by the Transition Improvement Grant (TIG), with assistance from the Wisconsin Community on Transition (WiCoT), to help CCoT teams build, strengthen, and sustain community partnerships that support successful postsecondary outcomes for youth with disabilities.

Whether a team is just getting started or has been active for many years, the toolkit provides practical tools and resources to support team development, collaboration, data-informed decision-making, communication, and continuous improvement. These resources are designed to help teams stay focused on improving outcomes for youth and young adults with disabilities.

CCoT Team RESET PowerPoint

The CCoT Team RESET PowerPoint is designed to help teams pause, reflect, and refocus their work throughout the year. This resource guides teams through reviewing current practices, identifying priorities, celebrating progress, and determining next steps. Whether used during annual planning, after team membership changes, or when teams need to re-energize their efforts, the RESET PowerPoint provides a structured process to help teams stay aligned, focused, and moving forward toward improved outcomes.
