Transition Planning 2025-2026
The Transition Improvement Grant (TIG) will offer two professional development online courses focused on transition planning. It is recommended that participants new to transition planning begin with the Introduction to the Postsecondary Transition Plan (PTP) course, as it provides a comprehensive overview of the PTP.
The Introduction to Postsecondary Transition Plan is a Self-Paced Micro Course. This means it is short, focused courses targeting specific skills or knowledge areas, perfect for professionals looking for quick, actionable insights they can apply immediately. These courses provide flexibility for learners who want to enhance their expertise in targeted areas without the time commitment of a full-length course.
A NEW Self-Paced Macro Course “Planning with Purpose for Post-Secondary Success” will be coming this October! This means it is a comprehensive, in-depth learning experience designed to provide a broad understanding of critical transition topics. This course will allow participants to dive deeply into the subject matter, covering extensive material that supports professional growth.

Course Description:
Gaining a solid understanding of the Wisconsin PTP application will provide educators with the tools and insights to create meaningful PTPs with the students they serve. This introductory course includes procedural requirements, culturally responsive practices, best practices in PTP writing, transition resources, and application shortcuts to increase efficiency when developing a PTP. This coursework is an online, self-paced format that includes learning content, reading opportunities, videos, and a final knowledge check. The course will assist in building the capacity to create effective and compliant transition plans with students with IEPs. This course has been created by the Wisconsin Transition Improvement Grant.
