The Capacity Building Event is designed to bring together transition partners, including educators, agencies, organizations, youth and their families, and community transition partners, to increase knowledge of effective transition-focused practices and resources through the following workshops:
Transition Partnerships Workshop – October 29th 1:00 – 4:00 p.m.
This workshop is designed to build robust foundations, refine processes, and implement best practices for supporting youth with disabilities as they navigate their journey from school to post-school life. The workshop will feature a combination of key speakers, panel discussions, and networking opportunities providing a platform for learning, sharing, and collaborating on innovative ideas and solutions.
Community Transition Innovation Planning for County Communities on Transition (CCoT) Teams – October 30th 8:30 a.m. – 3:30 p.m.
This workshop focuses on learning and applying that learning to the county context related to transition-age students and youth within your communities. Participants will engage in strategic planning utilizing data-based decision-making at the local level, fostering collaborative partnerships and service delivery. Through an activity selection process, action planning, and interagency agreements, each team member’s voice will be heard and honored as the team engages in the Community Transition Innovation Planning process. If you are currently not a member of a CCoT Team, please join us and we will connect you with your county transition partners.
